Plan for Extended Contact Hours and Deadline Schedule

 
The professional development sessions sponsored by the Southwest Education Alliance usually provide six contact hours that can be converted to .6 renewal credits by your school district.  However, the sessions labeled “6 + 4 Plan” provide the opportunity to earn four additional contact hours totaling 1.0 renewal credits.   The participants of these sessions are eligible to earn  four additional contact hours by completing the following activities: 
 
1. Read the presenter’s book/materials made available to all participants.
 
2. Using the attached form and rubric as a guide, create a high quality document substantiating your application of the workshop knowledge/skills.  This may be submitted online or you may mail or fax the document to the Southwest Education Alliance.  The due date for this submission is within four weeks of the workshop and is noted at the top of the form.

Log onto www.SouthwestEducationAlliance.com, choose the Extended Contact Hour link and
fill out the on-line version.  Please save a copy of the document for your records.
 
· Mail or Fax
      Call 704-687-8823 or 704-687-8821 to request a copy of the survey.  Fax survey to
      704-687-2110.  Please save a copy of the document  for your records.
 
By completing the above activities, the workshop participant will receive a certificate denoting the completion of ten (10) hours of “High Quality Professional Development.”   The certificates will be sent out within four weeks of the due date.  If you do not receive the certificate within that time frame, please call 704-687-8823.   In turn, the participant can apply to their school district for 1 renewal credit.  Please keep a copy of the completed document for your records.
